Peoria High School
Child Oriented Occupational Program
COOP stands for Child Oriented Occupational Program. The purpose of this program is to give “on site job training” to high school students who are interested in an occupation that involves young children. The students selected for this program will have had Child Development, which includes interaction with the COOP. They will be given exposure to day care center management, curriculum development, lesson planning, discipline, health and safety and AZ Day Care Regulations under the direction of certified and experienced instructors.
The COOP is licensed by the AZ Department of Health Services, located at 150 N. 18th Ave. Suite 400, Phoenix, AZ
85007. They may be reached at (602) 364-2539. Inspection reports may be viewed there. Inspection reports are also available in the COOP office. Please see Mr. Fishencord if you would like to review them.
ENROLLMENT:
Preschool children enrolled in the program should be 4 years of age by of September 1 and toilet trained. Enrollment takes place in March with space limited to 26 children. The boy/girl ratio is considered when registering children.
TUITION:
The cost for the program is $28 per day. We depend totally on tuition to pay salaries and purchase equipment and supplies. To balance our budget, it is necessary to charge for days absent. However, if your child is absent for more than three consecutive days because of illness, credit will be given on the fourth day and thereafter. NO credit will be given for absences due to vacations or reasons other than illness.
Tuition is due at the end of the week. You are welcome to pay in advance, but students will not be able to attend if they have any outstanding balances. We are requesting that all tuition payments be made by cash, cashier’s check or money order. WE ARE UNABLE TO ACCEPT PERSONAL CHECKS. Credit and debit cards can be used for payment, but those payments must be made in the PHS Bookstore during school hours. We will not be able to process those payments in the COOP.
HOURS OF OPERATION:
6:45 a.m. – 5:00 p.m.
The COOP student teachers will work with your children from 7:20 a.m. to 2:20 p.m. In order for your child to have
the full COOP experience we ask that they are at school by 8:00 a.m. This is the time that class activities will start. We follow the Peoria District Calendar for holidays. Our student interns will work with your children from 2:20 – 5:00.
There is a supervising adult with the children at all times.

Please notify us if your child is going to be absent. If your child is ill with a contagious disease (chicken pox, strep throat, pink eye, etc.), please notify the COOP immediately. The following list of symptoms suggests an oncoming or existing illness. An ill child shall not be admitted. If a child becomes ill after being left, a parent will be called to pick up the child. That child will remain isolated from the other children until his/parent or authorized adult arrives.
1. Red, water eyes.
2. Nasal discharge.
3.Coughing.
4.Sneezing
5.Rash on body, arms, legs, neck or face.
6. Flushing and excess warmth of skin.
7. Listlessness and/or irritability.
8. Complaint of headache, earache, or aching limbs.
9. Scratchy throat.
10. Nausea.
11. Vomiting.
12. Diarrhea.
LIABILITY INSURANCE is carried by the Peoria Unified School District.
FIELD TRIPS:
Throughout the school year we will be taking various field trips. Every child must have a signed permission slip to participate on the trip. The district will provide transportation. All participants will wear their COOP t-shirt so that it is easier to keep track of everyone in the group.
VISITORS ARE WELCOME:
You are always welcome to come visit your child during the day or spend lunchtime with him/her. We ask that you report to the front office first and get “cleared” with a visitor name tag.
BIRTHDAYS:
Birthdays are special days for children. Your child may want to share with the class on this special day. Any food brought in must be purchased from a bakery or store. State law prohibits homemade products.
DISCIPLINE POLICY:
The purpose of discipline is to help a child learn appropriate behavior and develop inner controls. Our staff will define, model, and encourage appropriate behavior. Whenever possible, the center personnel will explain why a particular behavior is not allowed, then suggest an alternative behavior. The child will be assisted in engaging in acceptable activities. Corporal punishment shall be prohibited.
We have a district wide discipline policy in each of the COOPs. If there seems to be chronic problems with a child’s behavior, it will be documented and a conference will be called with the parents to see how they might best be solved. If the behaviors continue, they will be documented and another conference will be called. At this point, if nothing works to change the negative behavior, the parent may be asked to find an alternative to the COOP for childcare. Most
children do very well in this program, but not all, and leaving him or her in a program in which there isn’t much success benefits neither the child nor the program.
ARRIVAL AND DEPARTURE:
Always bring your child to and pick up your child from the classroom. We are responsible for the safety of your child and can only be so when we are aware of his/her arrival and departure. You must sign in your child on arrival and you must sign your child out when departing.
All parents are required to have on file a list of names and phone numbers of people other than yourself who have
permission to pick up your child. Picture I.D. will be required from the person who picks your child up. If someone other than yourself is picking up your child, you must inform us of the special arrangements. You may add names of authorized persons to your child’s emergency card. Additions need to be done in person, not over the phone.
Please be considerate and pick your child up on time. There is a $10.00 late charge for every 15 minutes
past 5:00 that your child is not picked up. This should be paid in cash to the p.m. coordinator.

*The gates will remain closed and locked during school hours. If you are dropping off or picking up any time after 8:15am and before 2:20pm, you are to report to the main office and check in.
COOP Parking passes are required at drop off and pick up, two will be given to each family. Any additional passes can be purchased in the COOP.
CLOTHING:
Please dress your child in comfortable play clothing. Do not dress your child in clothing they are afraid to soil.
This limits play and creativity. Your child will be using paint, glue, and markers daily. We try very hard to prevent contact of those items with your child’s clothing. We cannot be responsible for any damage done to your child’s
clothing. If your child comes home with tempera paint on his/her clothing, try to remove it by rubbing bar soap directly on the soiled area. Rinse with cold water and repeat if necessary.
Shoes are extremely important and should be the kind that can run and climb. Non-skid soles are the best.
We ask that all children have a second change of clothing left at school, including underwear and socks, in case
of accidents. Please make sure the extra clothing is appropriate for the weather. Please have the extra clothing as well as their jackets marked with their names.
DAILY ACTIVITIES:
We gear our day around 5 class periods, with each period instructing the children in a different academic or social area. They are; music, art, science, language, physical education, math, and organized play, technology as well as free playground time.
REST TIME:
Rest time is mandatory for all children.
They are not required to sleep, just rest. Rest time is from 11:45am-1:00 p.m. Each child has his/her own rest cot. We ask that you provide a small blanket or towel for covering, one that will fit in their cubby. Your child is welcome to bring a comfort toy for rest time, one that fits in the cubby. NO PILLOWS.
HEALTH AND SAFETY:
Every precaution is taken to protect the children from accidents and illness. Our school nurse is available from 7:00 a.m. to 3:00 p.m. in case of emergencies. Please notify us immediately if your phone number, address,
doctor, or other emergency numbers have been changed. An emergency is not the time to find out that you cannot be reached. All medication must be brought to the COOP in the original bottle clearly marked with your child’s name, the dosage and directions. You will be required to complete a medication form. If your child requires an over the counter medication, it must have your child’s name on it, along with a statement from the doctor regarding
the dosage to be administered. We are not allowed to use “stock” medication.
